During the Stay
Provide onsite personal care – including assistance with daily living activities such as bathing & hygiene, dressing and mobility.
Complex health & medical care – including nursing services, wound and medication management,
Support transport and appointments – accompanying clients to hospital visits, community activities and allied health sessions.
Coordinate additional services – such as meal preparation, equipment delivery, and allied health visits to the accommodation.
Emotional & Social Support
Offer companionship and reassurance, especially for those travelling away from home or dealing with complex situations.
Promote independence and choice, empowering individuals to feel safe and in control throughout their journey.
Ongoing Care
Continuity of care MediStays care teams can continue to a permanent living situation in disability housing.
Assist with ongoing visits, ensuring people attending regular appointments have access to a consistent care team for their visits.
