NDIS Medium Term Accommodation to support a change in circumstances  

Who: Roger (55 years old, Down Syndrome, Alzheimer’s Dementia, Parkinson’s Disease)  
Stay type: Rapid hospital discharge NDIS-funded MTA while awaiting SDA

About Roger:

Roger initially presented to the Emergency Department at Monash Health due to an altered conscious state as a result of his multiple comorbidities and degenerative conditions. Despite having previously been quite independent, Roger’s change in function was marked, and therefore discharge back to his group home was no longer possible. Supported by his sisters and niece, the aim for Roger was to find an appropriate NDIS funded Medium Term Accommodation (MTA) environment as a transition to new long-term SDA housing. 

NDIS Plan Goals for Roger:

Roger’s referral to MediStays was placed by his Monash team via our MTA Rapid Request form. Our specialist Care Navigator team worked closely with the Monash and Community care teams and Roger’s family to assess his needs and find a suitable property to support his hospital discharge. Collectively, our goal was to provide the best environment for Roger’s ongoing rehabilitation whilst awaiting his SDA funding approval.  

Alongside his care and physical requirements, Roger’s primary NDIS plan goal was to maintain as much independence as possible. He was also seeking support to continue engaging in his interests and hobbies, which include attending local football games.   

With extensive experience in hospital discharge transitions, our Lead Care Navigator Jayde identified our Carrum Downs property as the ideal location to achieve Roger’s goals. Firstly, the property offers Roger a spacious and private bedroom with his own TV for watching footy plus a roll-in ensuite ensuring his privacy and high level of self-care. This sun-filled property also offers large communal living areas to facilitate easy interactions with other young residents and visitors.  

Roger is also able to receive regular onsite sessions from his disability physiotherapist to support his change in mobility and to allow him to maintain a level of physical activity and fitness. Roger’s family and care team is also working alongside MediStays’ Care Navigators to begin increasing Roger’s community participation and to get him back to the activities he enjoys most such as swimming, in the context of his changed physical abilities.  

Reflecting on Roger’s progress at MediStays @ Carrum Downs

Our Carrum Downs property is providing Roger with an ideal environment to continue his rehabilitation and to build his physical strength and community engagement capacity. Having his family and physio team being able to visit onsite allows Roger to have continuity in his care in a peaceful environment where he is now settled while awaiting SDA.
